Patients can request copies of their medical record by calling the Medical Record Department at (919) 526-1717 and then choosing Option 4. Patients should leave their full name, date of birth along with the pertinent information as to where the records will be sent. Please allow 7-10 days for all requests for copies of medical records.
All requests for copies of medical records require a written request and a signed authorization by the patient before copies of records may be released. Our Staff have been requested by our Privacy Official to request identification (such as NC Drivers License) to respond to any request for copies of records.
Copying charges include the cost of supplies and labor. There is a charge of $.75 per page up to 20 pages and then $.50 a page per page 21-100+ pages. There will be an additional charge of $5.00 if these are old records requiring that our Staff retrieve them from our off-site facility where they are stored.
